TRICARE is the Department of Defense's worldwide health care program for active duty and retired uniformed services members and their families.
TRICARE Network Provider
The Louisville VA Medical Center (VAMC) is a TRICARE Network Provider, and a Tricare Primary Care Manager for Prime beneficiaries. The VAMC is open 24/7 and provides a host of specialty services. We provide care to all the Tricare Plans; Prime, Standard, Extra, Tricare for life, Reserve Select, and Prime Remote to name a few.
TRICARE Program
The Louisville VAMC's Tricare Program is very unique; we are the only VAMC with a Tricare Family Practice, dedicated to servicing Tricare beneficiaries. Our caring professional staff is unmatched, they will make you feel at home and welcome on each and every visit. The family practice is located at 1101 Grade Lane in Louisville at the Kentucky Air National Guard Base. The Office number is (502) 413-4635.
Urgent Care
The Tricare Family Practice is also home to an Urgent Care Center, another first for the VA. In an urgent situation, contact the number above or call the TRICARE Coordinator at (502) 287-6940 for a same day appointment or advice on how to handle the immediate situation. The "Prime" beneficiary will have to contact Tricare for an authorization to be seen at the Urgent Care Center.
Emergency Care
All TRICARE beneficiaries, in the event of a life-, limb-, or eyesight-threatening emergency, should call 911 or come into our ER at 800 Zorn Ave.
Specialty Clinics
Referrals to our specialty clinics will require a physician's referral. Please contact the coordinator below for assistance to insure that you get the necessary authorization.
Pharmacy
The VAMC is a Tricare Network Pharmacy; however, we can only fill prescriptions written by our staff of physicians.

CHAMPVA is a healthcare benefits program for:

dependents of veterans who have been rated by VA as having a total and  permanent disability,
-
survivors of veterans who died from VA-rated service-connected conditions, or who at the time of death, were rated permanently and totally disabled from a VA-rated service-connected condition; and
-
survivors of persons who died in the line of duty and not due to misconduct.
Under CHAMPVA, VA shares the cost of covered healthcare services and supplies with eligible beneficiaries.
CHAMPVA In-house Treatment Initiative (CITI)
The CHAMPVA In-house Treatment Initiative, otherwise known as CITI (pronounced city), is a voluntary program that allows for the treatment of CHAMPVA beneficiaries at VA healthcare facilities. The Louisville VA Medical Center participates in the CITI program. Choose The Louisville VAMC as your healthcare provider of choice!
Advantages of CITI participation?
From the CHAMPVA beneficiaries' perspective.no costs! An $80 outpatient visit to a private provider, for example, costs the beneficiary a minimum of $20 is cost sharing while the same visit to a CITI participating facility is free cost. Although numerous factors are at play in determining inpatient cost shares, the typical VA inpatient episode saves the beneficiary hundreds of dollars.
Start-up is easy!
Since there's no formal enrollment application process, facilities can initiate participation at any time. In addition, CHAMPVA beneficiary demographic and treatment information is available electronically.
